LPD Unable to Send Document - Unauthorized

This is a follow-up on a post I made in the general forum: "How do I recover from a holding state?".  I should have named it "How do I get LPD out of an unauthorized state?".

This is actually two problems.

CUPS crashes when printing to an LPD queue configured with banner page.  (I'm running CUPS v1.2b2 binary for Mac OS X Tiger.)  I include the crash dumps below.

The second problem and most pressing for me to fix is recovering after the crash.  On restarting the server, CUPS enters an indeterminable state of holding LPD jobs.  The message appearing in the system.log file is:

Mar 20 15:49:38 Print cups-lpd[514]: Unable to send document - Unauthorized

This unauthorized state will last for hours, during which nothing is logged as being completed, even IPP jobs that remain working.  After some hours the problem will go away and the back-log of jobs (cancelled or processed) show up.

Can you please clue me in as to how to get LPD out of this unauthorized state?
