Pass Format/Form to label printer

I am using version Red Hat 3.4.3-9.EL4 with CUPS, any suggestions on how I can pass a form would be appreciated.

I am able to print with the command lp -dMonTest formatfile testfile, however I need to be able to send the formatfile to the printer to change the printer setting format. I have looked into trying to put the formatfile via /etc/printcap, but that does not work. Is there a way I can just print via lp -dMonTest testfile, where it will automatically pick up the formatfile?

Any help on this would be greatly appreciated.
