client-error-not-authorized

has anyone found a solution for the "client-error-not-authorized" problem with 1.2.x?
It seems that the username "anonymous" is used if no user name is set in the request.  I can not find a way to have the application to ask for a user name, so it uses the default and always fails.  Below are two lines from the error log file.
