I've downloaded pdftops.pl and the conf file, but I'm unclear as to where to put them, although the conf file says it goes into the CUPS_SERVERROOT directory.

Did you install the pdftops.conf in /etc/cups ? and did you assert that
it contains
the statement

psPaperSize match

?

This directive is vital, as the default (when building xpdf) is set to
portrait letter.
You could try "-o pdf-paper=match" as an option to the print command.

As a last resort to nail the issue down, temporarily modify your
printers's URI to
file:/some_absolute_path_name
and enable printing to file in your cupsd.conf, restart CUPS, and post
an URL to the
generated file. And let me know which version of CUPS you are using.

PS: with my configuration, it prints OK.
