Your PostScript file does not conform to the Adobe document structuring
conventions, but claims conformance (the %!PS-Adobe-2.0 line at the top).
The two major problems are having the %%EndComments line in the wrong
place and changing the graphics state outside of a page definition. The
printer option commands are resetting the graphics state, leading to your
blank page.

The quick fix is to change the first line to just "%!" or add a line above
it with that, e.g.:

    (echo '%!'; cat filename.ps) | lp
