[cups.general] cups 1.2.3 remote print jobs only print cover page? log excerpt attached

So, when I print a job remotely (LPD or IPP) to our CUPS server, if  
there is no cover page defined then the job prints.  If there is a  
cover page defined, only that prints.  Jobs originating locally print  
fine.  The attached log extract appears to show cupsd setting up to  
process the second file (i.e. the real print job) but not actually  
doing anything with it.

In addition to the log extract:  I set up a backend which copied what  
was passed to it to a temp file so I could see what exactly was being  
sent to the printer.  This confirmed what the log told me:  CUPS  
appears to process the second file, but in fact apparently does  
nothing with it and it never appears in the output.

Clues?  I'm unable to interpret what, if anything, is going on/wrong  
from the log.
