pdftops prints document shifted up and along 2 inches

So are you also printing on A4 *paper*?

Try these commands:

 (a) for printing on Letter size Paper:
     lp -d printername -o PageSize=Letter -o fitplot /path/to/pdf

 (b) for printing on A4 size Paper:
     lp -d printername -o PageSize=A4 -o fitplot /path/to/pdf
