Mac OS X is getting many "-60002 authorization reference is invalid" error
messages in error_log. The attached patch fixes this (along with general
cleanup) by:

- Moving the authorization reference from a thread global to the http
structure.
- Not closing the auth_ref at the bottom of cupsDoFileRequest().
- Have http_send() clear authstring for both Kerberos and AuthRef
authentication types.
