Printer keeps going disabled

I have a Centos 4.4 server running the stock CUPS 1.1.22.  I have over 40 printers being shared out to several hundred workstations.  Everything is working fine except for one printer...

This one printer keeps going disabled.  I can login to my server, re-enable it, and all goes good again.  About a week later, a user calls and says they can't print.  I login again and the printer is disabled again.

There has to be a reason why this print queue is stopping.  But, I don't have anything in my error log.

One other note, the printer itself is a Laserjet 5si.  We have a special requirement for this printer where I configured two queues for it.  The one queue name forces everything to letter paper where the other queue forces everything to 11x17.  Is it possible there is some sort of collision going on when two people print to differing queues at the same time?

Any thoughts would be greatly appreciated.
