rastertohp positions the output at the upper-left corner of the
PCL logical page...

That's a driver issue...

Yes.  All you get from Ghostscript (or any other RIP filter) is
a raster image that is extracted from the ImageableArea defined
in the PPD.  The driver, printer, and PPD generally have to have
matching notions of what the imageable area is... :)
