As for the patch size, generally anything over a few dozen lines
that isn't composed of obvious typo fixes probably will require
copyright assignment or licensing to Apple Inc.

If you have specific legal questions, send them to cups-info at cups.org
and I'll forward to the appropriate people at Apple.

Typically yes.  There have also been contributions provided under MIT
and New BSD licenses (which allow Apple to redistribute as needed
while the author retains the original copyright...)
