Custom page size problem -> always printing to A4

Please tell in detail what printer you are using and post
(an URL to) the printer's PPD (the one found under 
/etc/cups/ppd, which reflects the defaults for the printer),
and let us know how the printer is configured in CUPS
(what backend etc.).
