The PPD file uses CUPS custom PPD feature.

*Password None/None: ""
*Password 4001/4001: "xxx"
*Password 4002/4002: "xxx"
*Password 4003/4003: "xxx"
*CloseUI: *Password

*CustomPassword True/Custom Password: "@PJL SET
JOBPASSWORD2="\1"<0A>"
*ParamCustomPassword Password: 1 passcode 4 8

When user set default Password value through CUPS web interface (1234, for
example), lpoptions -l will show the following:

Password/Locked Print or Doc Server Password (4-8 digits): None 4001 4002
4003 *Custom.PASSCODE

This is the desired behavior. However, if user set default Password using
lpoptions command line, 
george at ubuntu:~/Project$ lpoptions -p 3500PXL -o JobType=LockedPrint -o
Password=1234

The following will be shown with lpoptions -l 
Password/Locked Print or Doc Server Password (4-8 digits): *None 4001 4002
4003 Custom.PASSCODE

None is marked as default, which is a defect.
