Read the following manpages:

subscriptions.conf
mailto.conf
notifier

To build your own notifier, you may hack a script (Perl, Python or whatever) 
around the testnotify utility.

If you need a (nearly) realtime monitoring and/or watch certain limits, I'd 
recommend to install the open source nagios systm in addition (see 
www.nagios.org).
