Unsupported format "application/vnd.cups-banner".

If you go through the mime types and whatever the final type that your ppd needs, there is no path to get from application/vnd.cups-banner to whatever the final type your PPD needs.  You'll either need to change your PPD or add another mime.convs to complete the chain.
