[cups.general] /usr/libexec/cups/backend/usb failed

This sounds like a bug in the USB backend.  I don't know how you would enable core dumps on Gentoo, but that's what you need (plus gdb) to provide the info needed to diagnose this issue.

You could also try the current USB backend from CUPS 1.6.x which is significantly improved from 1.5.x...
