Is your cups running on a Linux system and you are using the cups-filters
package from open printing.org?
If yes, those filters obviously are ignoring the CutMedia stuff.
Instead, install and try the filter
gstoraster
from 
http://sourceforge.net/projects/cupsaddons/files/filter-gstoraster <http://sourceforge.net/projects/cupsaddons/files/filter-gstoraster>
This filter *does* deal with that option from the PPD.
