Solved.  The hylafax installation defaults wrote the sendfax binary to 
/usr/local/sbin and
looking at /var/log/cups/error_log it was looking for hylafax in 
/usr/sbin.  I created a symlink and it's working now.  Should have 
checked the error_log before I posted.
